Do you believe you should have received a 1099 from Blue Cross and Blue Shield of Texas (BCBSTX) and have not received one?

You may request a duplicate 1099 by sending your request to the BCBSTX Finance Department using the following:

• Fax #: 972-766-0898

Your request should include the following information:

• Physician/Group Name
• Tax Identification Number
• Correct Mailing address
• Contact Name and Contact Number

Questions regarding your 1099 should be directed to our Finance Department at 972-766-6623.

Note: If you have less than $600.00 in claims payment, the IRS does not require that we supply you with a 1099.
